MEP coordination in healthcare: why infection control zoning changes the clash review.

A clash that's cosmetic in an office tower can be a compliance failure in a hospital. Healthcare MEP coordination needs its own review sequence.

MEP coordination on a healthcare project follows the same geometric principles as any other building type, but the consequences of getting it wrong are different in kind, not just in degree. A coordination failure in a critical care unit is not a rework cost — it is a potential compliance failure with infection control requirements that can delay opening, trigger a planning condition, or require physical remediation after the building is occupied.

The specific challenge is that infection control zoning creates a layer of requirements that are not visible in the model geometry. Two rooms that are physically adjacent may be in different pressure zones, with different ventilation supply and extract requirements, different surface treatment specifications, and different clearance standards for maintenance access. A duct that passes through the boundary between those zones without the correct damper assembly is not just a coordination miss — it is a ventilation failure.

We restructure the clash review sequence on healthcare projects to make zone boundaries an explicit coordination check, not a discipline-specific responsibility. That means overlaying the infection control zone drawing on the federated model as a reference file and running clash checks against the zone boundaries themselves, not just against other building elements.

The practical consequence is that the MEP coordination report on a healthcare project has an additional section that does not appear on other building types: boundary checks, listing every service that crosses a zone boundary and confirming that the correct assembly is present in the model and coordinated with structure and finishes.

This adds time to each coordination cycle. It adds significantly less time than discovering the issue during commissioning, which is the alternative. The NHS projects we have worked on have all required this approach, and the contractors on those projects have been clear that receiving a model with zone boundary coordination already resolved is the single most valuable thing a BIM team can deliver on a healthcare programme.
